The Two Rivers Hospital Partial Hospital Program (PHP) is designed to meet the mental health needs of patients in acute crisis. PHP provides structured group therapy, individual therapy, and crisis intervention. This program targets acute psychiatric symptoms for patients at risk for hospitalization. It also provides an alternative for patients discharged from inpatient settings.
PHP provides short term, intensive mental health treatment. It offers structured, group therapy from 8:30 am to 3:00 pm, Monday through Friday. Transportation is available to those who qualify. Lunch is included.
Daily treatment sessions include grief and loss, relapse prevention, cognitive behavioral therapy (CBT), expressive therapy, relaxation therapy, family therapy, and anger management.
The multidisciplinary treatment team, headed by an in-house psychiatrist, helps create individualized treatment plans. Patients meet with their appointed psychiatrist several times, weekly. Other members of the treatment team include a registered nurse, a PHP manager, an expressive therapist, and licensed clinical social workers.
Discharge planning is a key component of PHP. Aftercare plans are developed by the treatment team to insure continued, ongoing progress, after discharge. Plans may include step down to the Intensive Outpatient Program (IOP). |
